OET vs IELTS for nurses is the first decision every Kerala nurse heading to the UK or Ireland has to make, because both regulators accept both tests. The UK's Nursing and Midwifery Council (NMC) and Ireland's Nursing and Midwifery Board (NMBI) each publish their own minimum scores, and the rules on combining attempts and test formats are not the same in the two countries.
This guide is for GNM and BSc nurses comparing the two exams before booking a date. It sets out the exact scores each regulator asks for, how OET and IELTS differ in content, where the sitting and validity rules trip people up, and which test is usually the faster route for a working nurse in Kerala.
1.The short answer: which test should a nurse take?
The decision usually comes down to two things: which format plays to your strengths, and how the score rules work for the country you are targeting. A nurse who handles case notes, referral letters and patient conversations every day already practises the core of OET. IELTS tests general academic English — essays on social topics, lectures, and reading passages unrelated to healthcare.
There is no "better" test in the regulators' eyes. Both are full routes to registration. Pick the one you can pass with the least wasted attempts.
2.NMC (UK) English requirements for nurses
The NMC's published minimums:
- IELTS Academic: reading 7.0, listening 7.0, speaking 7.0, writing 6.5. Paper-based and computer-based tests taken at a test centre are accepted, as is IELTS for UKVI.
- OET: you must take the Nursing version. Reading, listening and speaking at grade B (350 or above); writing at grade C+ (300 or above). OET on Paper, OET on Computer and OET@Home are all accepted by the NMC.
The writing relaxation (6.5 in IELTS, C+ in OET) is the single most useful detail for Kerala candidates, because writing is the sub-test that holds most nurses back.
3.NMBI (Ireland) English requirements for nurses
Ireland's scores look almost identical to the UK's, but the rules around them are stricter in three places: NMBI accepts only one test result (no combining attempts), the result must be under two years old, and it does not accept IELTS online, the IELTS One Skill Retake, or OET at home.
If Ireland is your target, plan to hit every band in a single sitting at a test centre. For a fuller country comparison, read our Germany vs Ireland nursing guide.
4.OET vs IELTS side by side
How the two exams compare on the points that matter to a nurse:
- Content: OET Nursing uses healthcare material throughout — consultations, case notes, workplace texts. IELTS Academic uses general academic topics.
- Writing task: OET asks for a single referral or discharge letter based on case notes. IELTS asks for a data description plus an argumentative essay.
- Speaking: OET is two role-plays with an interlocutor playing a patient or relative. IELTS is a three-part interview on everyday and abstract topics.
- Scoring language: OET reports grades (A to E, with scores up to 500). IELTS reports bands from 0 to 9.
- Beyond nursing: an IELTS score is also usable for study and many non-healthcare visa routes; OET is a healthcare-specific credential.
Neither test is shorter or "softer" — the difference is whether the content is familiar ground. Our IELTS vs PTE vs Duolingo comparison covers the wider test landscape if you are also weighing study routes.
5.Combining sittings, validity and formats that are refused
The NMC rule for combining two sittings: across the two tests you need one reading, one listening and one speaking result at the full minimum and one writing result at the writing minimum — and no score in either sitting can fall below the floor. For IELTS the floors are 6.5 in reading, listening and speaking and 6.0 in writing. For OET they are C+ in reading, listening and speaking and C in writing.
What both regulators refuse for IELTS: IELTS Online and the IELTS One Skill Retake (the NMC also refuses the IELTS Indicator). NMBI additionally refuses OET at home, while the NMC accepts it. Book a centre-based test unless you are certain your target regulator accepts the format.
The NMC also asks you to authorise it to verify your OET result before you submit your application — a small step that delays files when it is missed.
6.Which is easier for Kerala nurses?
A practical way to choose: take one honest diagnostic in each format. If your OET writing letter already reads close to C+ and your role-plays feel natural, OET is your route. If you write structured essays comfortably and your IELTS reading is strong, IELTS may cost you fewer attempts — and keeps more doors open if you later consider study.
What matters more than the label is the gap between your current level and the minimum. Writing is where most attempts fail in both tests, so plan your preparation around it.
7.A preparation plan that works alongside duty
The pattern we see working for nurses who are still on shifts:
- Diagnose first: a full mock in your chosen test before you pay an exam fee.
- Writing every week: two timed letters (OET) or two essays plus a task 1 (IELTS), each marked against the official criteria.
- Speaking twice a week: role-plays with a trainer for OET, timed interview practice for IELTS.
- Fixed class anchor: an evening or weekend batch you do not cancel after a long shift.
- Book when mocks are consistent: two mocks in a row at or above the minimum in every sub-test.
